We’re looking for a dedicated, inclusive, and resourceful Case Manager I or II to join our Aging and Adult Services (AAS) Division, supporting the Aging and Disability Resource Center (ADRC) within the Office on Aging.
In this role, you’ll play a key part in the Transition Services program-helping Medicaid members living in Skilled Nursing Facilities understand their options and access resources to return to community living. You’ll also provide education and outreach to facility staff, community organizations, and families throughout the region.
ResponsibilitiesWhat you'll be doing:
- Conducting individual and group counseling sessions with Medicaid members in Skilled Nursing Facilities and their support systems, explaining the Transition Services program.
- Scheduling and facilitating training sessions and outreach efforts with Skilled Nursing Facility staff and community partners.
- Initiating referrals to the Transition Coordination Agency and Case Management Agency.
- Maintaining accurate and up-to-date resource information.
- Providing information and assistance regarding available community resources.
- Documenting all client interactions and outreach activities for tracking and reporting purposes.

Depending on qualifications, this position could be hired as a Case Manager I or II.
Case Manager I $23.7125/hr - $28.4551/hr
- Equivalent to a Bachelor's Degree from an accredited college or university required. Combination of education and/or experience may be substituted on a year for year basis.
- Possession of a valid Driver's License by date of hire required.
- Complete training and/or certification as mandated by the assigned program required.
Case Manager II $26.0838/hr - $31.3006hr
- Equivalent to a Bachelor's Degree from an accredited college or university required. Combination of education and/or experience may be substituted on a year for year basis.
- One (1) year of experience in a directly related field or in the performance of similar duties and responsibilities required.
- Possession of a valid Driver's License by date of hire required.
- Complete training and/or certification as mandated by the assigned program required.

Supplemental InformationThe typical schedule for this position is Monday-Friday, 8- 5 pm. This position is a hybrid position and will require at least one day/week in the office. You will be required to have adequate internet access and space for remote work. Larimer County may provide an annual remote work stipend to assist with equipment and office supply needs for remote workspaces. This p osition will be located at one of the following three locations: 1501 Blue Spruce Drive, Fort Collins, CO 2555 Midpoint Drive, Fort Collins, CO 200 Peridot Avenue, Loveland, CO
