    HEALTH AND HUMAN SERVICES AGENCY
    The El Dorado County Health And Human Services Agency is committed to transforming lives and improving futures for the citizens of El Dorado County.     PUBLIC GUARDIAN
    The Office of the Public Guardian ensures the physical and financial safety of persons unable to do so on their own, and when there are no viable alternatives to a public conservatorship. Probate conservatorships are primarily established for frail adults who are unable to provide for their own personal needs for physical health, food, clothing and/or shelter, cannot manage their own finances or are subject to fraud or undue influence. LPS conservatorships are established to arrange mental health treatment and placement for people who are gravely disabled and unable to provide for their food, clothing, shelter and treatment needs as a result of a mental disorder. Services are defined and directed by the Superior Court.

    COMMUNITY SERVICES DIVISION The Health and Human Services Agency’s Community Services Division for over 44 years, the Senior Nutrition program of El Dorado County has been promoting and maintaining the health and wellness of older adults in our community through the provision of a daily nutritional and satisfying lunchtime meal served in a group setting or delivered to the home. Meals contain at least one-third of the Recommended Dietary Allowance. The program offers more than just a hot meal, and is committed to promoting better health with improved nutrition, encouraging socialization, and providing access to supportive services. Congregate dining is available weekdays at seven locations throughout El Dorado County for individuals who are 60 years of age or older. Home-Delivered Meals are available for individuals who are 60 years of age or older and who are homebound and unable to shop for their food or prepare their own meals, on a temporary or permanent basis. Dedicated, caring volunteers deliver weekday meals and check on the wellbeing of each participant.

    THE OPPORTUNITY
    Provide various support to the administration and operation of specialized programs to meet the needs of participants in the community or social service programs.

    The selected candidate will have the opportunity to:

    • Receives and reviews program applications to determine eligibility and certifies clients for program services; counsels and advises applicants based on their needs and makes appropriate referrals to other community services agencies as applicable.
    • Issues program services certificates and vouchers and maintains records of such documentation and awards.
    • Performs program implementation including the scheduling of trips, collection of fees, reservation of transportation, and the promotion and oversight of scheduled program activities.
    • Provides information about resources, programs, service and entitlements, Medicare, medical and other programs which exist under Human Services.
    • Provides basic counseling about aging, nutrition, and low-income concerns and assists clients with independent living and long-term care matters.
    • Evaluates progress of program participants by assessing health, weight, hematocrit/hemogloblin measures and similar parameters.
    • Maintains liaison with co-workers, community groups, government agencies and other entities to ensure maximum usefulness of programs.
    • Maintains accurate files and records related to contacts with program participants.
    • Performs publicity and community outreach for assigned programs; may speak to various groups; conduct group classes on various program topics such as prenatal, infant, child and family nutrition.
    • Performs various office support work such as typing and filing; maintains fiscal and statistical records related to programs; compiles various reports.
    • Attendance and punctuality that is observant of scheduled hours on a regular basis.

    MINIMUM QUALIFICATIONS: Human Resources will assess your application to determine if you are minimally qualified using the following recommendations. Where college degrees and/or college course credits are required, degrees and college units must be obtained from an accredited college or university. Courses from non-accredited institutions will not be evaluated for this requirement.

    Education:Equivalent to possession of an Associate of Arts degree with major coursework in social services, psychology or a related field,

    Experience:
    One (1) year of experience in eligibility determination, counseling or intervention or in office sup port work which has included interpreting and explaining complex rules and regulations. Relevant experience may be substituted for the education on a year for year basis.

    Licenses and Certifications:

    • Possession of, or ability to obtain and maintain, a valid California or Nevada Driver's License and a satisfactory driving record.

    Public Guardian Ideal Candidate Statement

    The Office of the Public Guardian safeguards the physical and financial well-being of individuals who are unable to care for themselves and have no other viable alternatives for support. These services are mandated and overseen by the Superior Court, which appoints the Public Guardian to act as conservator in cases involving the person, the estate (probate matters), or both.     The ideal candidate for the Senior Nutrition Program Assistant position is a person who is comfortable working in a fast-paced environment, enjoys teamwork, is dedicated to public service, and has a passion for working with older adults. The Program Assistant is responsible for conducting intake and eligibility; assessing applicant limitations with Activities of Daily Living (ADL); screening for nutritional risk; coordinating volunteers; conducting outreach; answering phones; preparing deposits; developing outreach and informational materials; supporting and processing volunteer mileage reimbursement; and developing materials and spreadsheets for data tracking and quality improvement efforts. On a daily basis, this rewarding position interacts with office support, kitchen staff, volunteers, meal participants, and families, to provide a high level of customer service.
